You are not alone, Stephanie. Almost all singers have stage fright (a.k.a. performance anxiety) -- just to differing degrees. There are many good books out that can help you with this. "A Soprano on Her Head" by Eloise Ristad is one. I am convinced that being a good singer is about 30% vocal technique and 70% psychology! We all have to work on our heads to remove the blocks and obstacles that keep us from performing freely. Don't think you are the only one, and don't be afraid to seek the help you need. A good therapist can help, too, if that's a possibility for you. Good luck, and enjoy your first-back voice lesson!
