The pattern described - "ticcy" while not engaged in a task, but "normal" when engaged is apparently not at all uncommon among Touretters.
There are a couple of marvelous stories in some of Dr. Oliver Sacks' books.
I don't have 'em here, so I may be remembering cross-eyed, but check out "The Man Who Mistook His Wife For A Hat" for one, and "An Anthropologist On Mars" for a couple others, including the story of a surgeon who has Tourette's. Same pattern - he's hooty and ticcy and occasionally a bit scatological in "normal" life, but absolutely rock solid in surgery.
